Apply for a driving theory test application form
You can use the driving theory test postal application form to book by post if you can't use the online or telephone booking services.
How to use this service
Download the application form from the link below and fill it in.
You can pay for your test by cheque or postal order, but cash payments aren't accepted. You should make your cheque or postal order payable to 'Driving Standards Agency'.
Send your completed application form, with payment, to: Driving Standards Agency, Booking Department, PO Box 381, Manchester M50 3UW.

It's quicker and easier to book your driving theory test online. When you book online you can see all the available appointments and choose one that suits you.
