Hypoglycaemia and driving

Hypoglycaemia is a condition that you need to tell the Driver and Vehicle Licensing Agency (DVLA) about. Find out which questionnaire you need to complete for the driving licence you hold.

Car or motorcycle driving licence holders

To tell DVLA please download the medical questionnaire 'DIAB1' and send it to DVLA.

For help and guidance on filling in the DIAB1 questionnaire, please download the leaflet ‘A guide to filling in your DIAB1 medical form’.

Bus, coach or lorry driving licence holders

To tell DVLA please download the medical questionnaire 'DIAB1V' and send it to DVLA.

For further information for drivers who wish to apply for C1 or C1E entitlement, please download the 'INS186' leaflet.

Where to send your completed questionnaire

You can send your completed questionnaire to DVLA by fax or post.
