Giddiness (recurring) and driving

If you experience recurring episodes of giddiness you will need to tell the Driver and Vehicle Licensing Agency (DVLA) about it. Find out which questionnaire you need to complete for the driving licence you hold.

Car or motorcycle driving licence holders

Giddiness - vertebrobasilar

To tell DVLA please download the medical questionnaire 'B1' and send it to DVLA.

Giddiness - other

To tell DVLA please download the medical questionnaire 'DIZ1' and send it to DVLA.

Bus, coach or lorry driving licence holders

To tell DVLA please download the medical questionnaires 'B1' and 'FEP1V' and send them to DVLA.
