Diabetes on insulin is a condition that you need to tell the Driver and Vehicle Licensing Agency (DVLA) about. Find out which questionnaire you need to complete for the driving licence you hold.
To tell DVLA please download the medical questionnaire 'DIAB1' and send it to DVLA.
