Blackouts and driving

Blackout is a condition that you need to tell the Driver and Vehicle Licensing Agency (DVLA) about. Find out which questionnaire you need to complete for the driving licence you hold.

Car or motorcycle driving licence holders

Blackout: cough syncope, provoked syncope or unknown cause

To tell DVLA please download the medical questionnaire 'FEP1' and send to DVLA.

Bus, coach or lorry driving licence holders

Blackout: cough syncope

To tell DVLA please download the medical questionnaires 'FEP1V' and 'DIZ1V' and send them to DVLA.

Blackout: provoked syncope

To tell DVLA please download the medical questionnaire 'FEP1V' and send it to DVLA.

Blackout: Unknown cause

To tell DVLA please download the medical questionnaire 'FEP1V' and send it to DVLA.

Where to send your completed questionnaire(s)

You can send your completed questionnaire(s) to DVLA by fax or post.
