Acoustic neuroma is a condition that you may need to tell the Driver and Vehicle Licensing Agency (DVLA) about. Find out which questionnaire you need to complete for the driving licence you hold.
If you hold a car or motorcycle driving licence and suffer from acoustic neuroma, telling DVLA will depend on whether you have disabling giddiness.
Disabling giddiness
If you do not suffer from disabling giddiness - you do not need to tell DVLA.
If you suffer from disabling giddiness - you need to tell DVLA.
To tell DVLA please download the medical questionnaires 'B1' and 'DIZ1' and send them to DVLA.
If you hold a bus, coach or lorry driving licence and suffer from acoustic neuroma - you need to tell DVLA about your medical condition.
To tell DVLA please download the medical questionnaires 'B1' and 'DIZ1V' and send them to DVLA.
You can send your completed questionnaires to DVLA by fax or post.
