Apply for a vehicle registration certificate V5C (log book) - V62
Use form V62 to get a vehicle registration certificate (V5C) (formerly known as a log book) if the original has been lost, stolen, defaced, destroyed or if you have not received the certificate for your new vehicle.

Download the form from the link below and fill it in. The form includes useful guidance notes and information including where to send the form.
Important
Please ensure that you fully complete and sign the V62, if you do not it will be returned to you. This will delay your application.
